Most active corporations, LLCs, LPs, LLPs, business trusts and professional associations — domestic and foreign — must file once a year. The report lists the name, jurisdiction, entity number, registered office, principal office, at least one governor and principal officers (if any). No financial information.

Who does not file
Fictitious names, general partnerships that are not LLPs, authorities, name reservations, land banks, financial institutions and credit unions, name registrations, trademarks, and insignias and marks. Associations shown as inactive (dissolved, terminated, merged out, withdrawn, cancelled) do not file.
What the report asks for (DSCB:15-146)
- Name of the association
- Jurisdiction of formation
- Entity/file number issued by the Department of State
- Registered office in Pennsylvania — a street address, or the name of a commercial registered office provider (CROP) and county of venue — and any change to it
- Principal office address — and any change to it
- Name of at least one governor
- Names and titles of the principal officers, if any
No financial information is required, and the information must be current as of the date the report is filed — not as of the prior year.

Corporations must have a president, a secretary and a treasurer (one person may hold several offices); the president and secretary must be natural persons. Other associations list officers only if they have them.

The first report is due the year after formation or registration. Pennsylvania mails a postcard reminder to the registered office at least two months before the deadline — but not receiving it does not excuse the filing. Businesses using a commercial registered office provider get no postcard; the provider receives a list instead.
Other filings it does not replace
- Certificate of Annual Registration — still required by April 15 for LLPs, LLLPs and restricted professional companies.
- Federal BOI reporting — a separate federal matter handled by FinCEN; it has no effect on the Pennsylvania Annual Report.
