Electronic Signature and Records Consent

1. Your consent

When you check the electronic-signature box and type your name in our authorization step, you agree to sign electronically. Your typed name is your electronic signature and has the same legal effect as a handwritten signature under the federal E-SIGN Act and Pennsylvania’s Uniform Electronic Transactions Act.

2. What you sign

You certify that the information provided is true, accurate and current; that you are authorized to act for the business; and that you authorize Fire Up LLC (doing business as File PA Annual Report) to prepare and submit the Pennsylvania Annual Report to the Pennsylvania Department of State on behalf of the business.

3. What we record

Your typed name, the capacity you selected, the date and time, the version of the authorization text, your IP address and browser details, and the order it relates to.

4. Electronic records

You agree to receive order confirmations, notices and filed documents electronically at the email address you provide and on your private order page. You need a device with a web browser, an email account, and software that opens PDF files.

5. Paper copies and withdrawing consent

You may request a paper copy of any record, or withdraw consent before we file, by emailing connect@fireup-llc.com. Withdrawing consent before filing cancels the order with a full refund. Keep your email address up to date by contacting us.
